Priority: 1st choice alternative	Medication: pyrimethamine po loading dose 100mg, 50mg 1dd + azitromycine po 250mg 1dd + folinezuur po 15mg 1dd	Remarks: Behandelduur 4-6 weken gevolgd door herevaluatie oogarts. Niet tijdens zwangerschap.
Priority: 1st choice alternative	Medication: pyrimethamine po loading dose 100mg, 50mg 1dd + sulfadiazine po loading dose 2000g, 500g 4dd + folinezuur po 15mg 1dd	Remarks: Oplaaddosis sulfadiazine wordt 1dd voor 2 dagen gegeven Behandelduur 4-6 weken gevolgd door herevaluatie oogarts. Niet tijdens zwangerschap.
Priority: 1st choice alternative	Medication: clindamycine po 600mg 3dd	Remarks: Bij overgevoeligheidsreactie op sulfa component en contraindicatie azitromycine. Behandelduur 4-6 weken gevolgd door herevaluatie oogarts.

General comments

Behandel actieve toxoplasmose chorioretinitis bij niet-zwangere patiënten, niet-immuungecompromitteerde patiënten en patiënten die niet sikkelcel positief zijn, indien er sprake is van: lesies binnen of op de vaatboog of juxtapapillaire laesies; (visueel storende) vitritis.
Kleine perifere laesies bij immuuncompetente patiënten niet behandelen.

Start, bij matige tot ernstige vitritis, na 48 uur met prednison 40 mg/dag en bouw dit geleidelijk af vanaf dag 10 (liefst in vier weken tot 0).

Overweeg profylactische behandeling met de combinatie trimethoprim/ sulfamethoxazol (800/160 mg per twee dagen) indien er binnen vijf jaar ≥2 recidieven optreden of bij monoculi met centrale toxoplasmoselittekens in het goede oog.
