General comments
Dosering bij verminderde nierfunctie en dialyse wijkt bewust iets af van de KNMP Kennisbank, daar wordt het advies gegeven om bij hoge dosis (8-12 gram) verder te doseren obv spiegels. Aangezien dit in veel centra niet (tijdig) beschikbaar is, is hier gekozen voor een concreet doseeradvies.
