Comments:

Hemo dialysis: intermitterende hemodialyse: normale oplaaddosis, gevolgd door 25-33% van de totale dagdosis; alleen op dialysedagen, toedienen na de dialyse, bij restklaring kan doseren elke 24 uur nodig zijn.
