Hemo dialysis: normale aanvangsdosis, gevolgd door 33-50% van de totale dagdosis; alleen op dialysedagen toedienen, na afloop van de dialyse.

CAV / VVHD: normale aanvangsdosis, gevolgd door 50-66% van de totale dagdosis per 24 uur continu of in 2-3 doses.
